How are Joint Commission approved hospitals planning on handling the 2009 Standard MM.05.01.01 which reads:  "All medication orders are reviewed for the following:  Variation from the hospital's approved indications for use?"  Does your formulary also list what each medication is approved for in the hospital?  Anyone have any ideas on how to comply with this standard without reviewing and rewriting the entire formulary?
