Related to unit-dose packaging of medications, I'd like to hear from you if your hospital or clinic or pharmacy uses repackaging equipment (i.e. Fastpacker is one brand name) and how often, if any, adverse medication events have occurred that were caused as a result of using these repackaging machines, and if the problem was mechanical failure in nature or stemmed from human error.    Thank you.
