Has anyone else noticed that doctors often write incorrect discharge orders because they do not understand the little intricacies of the printed medication administration record (MAR) at their hospital?  Nurses and Pharmacist know all the problems with their own hospital's MAR but doctors are the ones writing the discharge orders and they often do not understand that they are often not reading the correct dose because they are not familiar with the problems of their MAR.  Example:  A local hospital stocks only Flovent 110mcg so a 220mcg dose appears on their MAR as Flovent 220mcg/2puffs.  I have noticed that doctores on discharge often write for Flovent 220mcg 2 puffs. I catch it at our hospital (Rehab) when they are transferred here but what about all the discharge Rx's that went out into the community with the wrong dose on them?  What can we do about this type of problem?
